Earn a wellness program point with a flu shot or other immunization
We are in the middle of flu season. Did you know that you can log your flu shot with
the wellness program and earn 1 point toward the $600 wellness rebate? Flu shots are
extra important this year to help reduce possible strain on the health care system
during the COVID-19 pandemic.
To log your vaccination:
- Login to preventioncloud.com
- Select "Activity Forms" and "Submit Form"
- Choose "Adult Immunization Attestation" from the drop down menu
- Fill out the corresponding date
- Click submit
