Qualify for FY16 wellness rebate by April 30

UA’s wellness program continues its positive growth. Last month we introduced the new on-site program manager from Healthyroads, Sara Rodewald. She has been working to identify wellness champions and to help transform the culture at UA into one of health and wellness.�

Building on last year’s successful rebate program, members wishing to qualify for a $600 rebate in FY16 must complete a Personal Health Assessment (PHA) and biometric screening, plus earn five additional credits before the April 30 deadline. Members can get credit for an annual exam, dental exam, adult immunizations, a vision exam and by being a non-user of tobacco products. Credits can also be earned by taking online courses, participating in telephonic coaching or a systemwide challenge through Healthyroads. The goal is to allow members flexibility in working toward their health goals.

Credit for exams and immunizations claimed through Premera will be automatically uploaded to Healthyroads via a data file feed retrospective to July 1, 2014. Vision exam data will be sent by VSP to Healthyroads. Most claims processed through December are uploaded now and the January claims will be uploaded soon. Remember, it could take up to 30 days for your provider to file a claim so be sure to schedule your exams or wellness visit soon.

Biometrics can be collected using a downloadable physician form, making an appointment with a Patient Service Center (PSC) in Anchorage or Fairbanks, or during on-site screenings that will be scheduled on all UA campuses in the coming months through a newly contracted vendor, Summit Health. More information on the biometric screening opportunities will be coming soon.

Like last year, employees and spouses or FIPs on the UA Choice health plan are eligible to participate.

Onsite Biometric Screenings Scheduled

We have scheduled several onsite screening events coming up to help you get your screening and earn your wellness rebate! See the schedule below for your location, then see the following information and link to sign up. Here are the screening events in date order:

Mat-Su Campus (Palmer)
March 4, 8:30 a.m. to 12:30 p.m.

Kenai Peninsula College (Soldotna)
March 5, 8:00 a.m. to 12:00 p.m.

Fairbanks Campus
March 9 and 10, 7:00 a.m. to 12:00 p.m.

Sitka Campus
March 11, 7:00 a.m. to 12:00 p.m.

Ketchikan Campus
March 13, 7:30 a.m. to 11:30 a.m.

Anchorage Campus
March 19 and 20, 8:00 a.m. to 1:00 p.m.

Northwest Campus (Nome)
March 27, 8:00 a.m. to 12:00 p.m.

Bristol Bay Campus (Dillingham)
March 31, 8:00 a.m. to 12:30 p.m.

To sign up for a screening appointment, please click here and register as a new user. This is a different process than used for onsite screenings last year, so everyone will be a new user. Please enter your name as shown on your Premera ID card as the same eligibility information is used for the wellness program.

Spouses and FIPs are welcome at the smaller locations, but since there are limited spaces and greater demand in Fairbanks and Anchorage, we prefer spouses/FIPs to use the Quest Patient Service Centers or health care provider forms in those locations.

If you can't make it to one of these screenings, and you are located in Fairbanks or Anchorage, you have another option with the Quest Patient Service Centers (PSC). You can schedule an appointment with a PSC by going to my.blueprintforwellness.com.

  • If you did this last year, use the username and password you set up as a returning user.
  • If this is the first time you've used this site, enter the registration key UOAremote and your unique identifier which is the first three letters of your first name plus your full last name, then the first four digits of your birthdate in MMDD order (for example, George Clooney would enter geoclooney0506).
  • Schedule your screening at a date and time that works for you

If you can't access a PSC and the onsite screenings don't work for you, you can schedule a wellness visit with your provider and request a routine blood work for your screening. Find the Health Care Provider form at Healthyroads.com or the UA benefits website.

Remember, the screening is one required part of the wellness program. You also need to take the Personal Health Assessment (PHA) at Healthyroads.com, and earn 5 credits for completing other required steps. Please see the benefits website or Healthyroads.com for more information.
